Forms and Parent Handbook

Forms in blue are required by all students on or before the first day of school

Forms in red to be completed if special
medical needs require them

If your child is to be given a general medication:

If your child has sunscreen or medicated cream or lotion:

If your child is to be given a medication for SEIZURES:

If your child is to be given ASTHMA medication:

If your child is to be given Auvi-Q, Epi-Pen or other medication for Allergy or Anaphylaxis:

Medication cannot be on site without proper accompanying paperwork. Please attach a photo of your child to his/her medication. Medication cannot be expired. All prescription medication must be in a pharmacy labeled container with child's name, dosage, frequency, and expiration date.
